Australian Resource Centre for Health Information (ARCHI)

Osteoporosis Model of Care –   A multidisciplinary team-based model. This care model is designed for patients over 50 years of age, presenting to the Emergency Department with a minimal trauma fracture. The patients are identified, referred and managed with intervention and follow up. Resources are included on the website.

Follow up of Fallers – This program aims to increase the uptake of falls prevention strategies for older people through phone follow ups within a week of discharge from the emergency department.

Physical Activity Leaders Network – The Physical Activity Leaders Network (PALN) is a support strategy designed to train and support community volunteers to deliver effective fall-safe physical activity classes in areas with low population density.

Reducing the Incidence of Falls in Residential Aged Care – An ongoing program to reduce the incidence of falls in a Residential Aged Care facility through identifying balance, mobility and strength problems in residents and tailoring individual programs to address these.

HIP HOP: Exercise class for adults following fractured neck of femur – This project aimed to review an exercise class for clients following a fractured NOF. The goals were to improve function post-op and address pre-morbid falls risk factors.

Falls Prevention – The Bega Valley Community Health Centre has established a program based on evidence based research that indicates a multidisciplinary approaches encompassing comprehensive identification of falls risks, assessment and targeted intervention can reduce falls and falls injury rates.
